UnitedHealth Group (UNH) executives warned on October 13, 2026, that cost trends for commercial benefits are exceeding initial expectations of 11%, citing high-cost specialty drugs and the No Surprises Act. The company raised its full-year guidance during the same earnings call, but investors remain focused on how rising medical costs could impact profits.
Each one-point increase in medical costs would cost UnitedHealth approximately $3.45 billion across all its health plans. This figure extends beyond the commercial plans where executives flagged the higher trend. UnitedHealthcare, the company’s health plan division, generated $345 billion in revenue during fiscal year 2025, making one percentage point of that revenue equal to the $3.45 billion estimate. Based on a raised earnings per share outlook of at least $19.50, estimated 2026 net earnings stand at roughly $17.7 billion. A one-point overrun would reduce that profit but not erase it; a cost increase of just over five points would equal the entire year’s projected profit.
Such a shift is not unprecedented. Executives reported the consolidated medical care ratio, a measure of payouts for care, at 88.4% for the third quarter of fiscal 2026, down from 89.9% a year earlier. The ratio swung by more than one point within a year, though that recent move benefited UnitedHealth. Executives also highlighted disputes under the No Surprises Act, noting arbitration awards can reach 6% of total medical spend in some regions.
UnitedHealth holds significant cash reserves to absorb potential overruns. The company generated $23.6 billion in free cash flow over the twelve months ending June 30, 2026. This amount easily covers a one-point cost bill, although dividends are expected to consume more than $8 billion in 2026. Operating income covered interest expense 5.6 times over the same period.
For 2027 plan pricing, executives assume current cost trends will continue. Parts of the business operate outside the health plans, such as Optum Rx, which brought in $155 billion in revenue in fiscal 2025, a 16.1% increase from the prior year. For the first time, more than half of Optum Rx membership comes from outside UnitedHealth’s own plans.
The Medicare annual enrollment period started during the same week as the earnings call. Management intends to provide further information about 2027 before the year concludes. UnitedHealth stock has experienced sharp volatility previously, falling 46% from its peak to its low between February and June 2025, compared to a 19% drop for the S&P 500. The next quarterly report is expected around January 25, 2027.
